I always use the Roman Empire as a criterion to determine this. If your country/race falls outside the borders of the Roman Empire, then your country/race is mostly irrelevant and exists only to fill the background like an NPC. There are some exceptions of course like Germany, Russia (the western part), Iran and the Gulf countries.
