The target selected by Mangione was a rich dude that became rich denying access to medical services to poor people. He was one of these normies that get elevated in society for their ability to make more money fucking the ass of paying customers.
Basically someone pays insurance, and when he has an issue goes to the doctor, some asshole lawman will tell him "Im sorry youre not entitled to this service despite you payed for it, fuck you" and he will pull some trick or clause in the contract to deny medical service.
Well, Luigi had enough of that bullshit. He took matters into his hands, and he killed a guilty CEO. The message is clear: denial of healthcare is a declaration of war, and Luigi responded accordingly. Luigi was right, Luigi was a hero, and he was memed with the Luigi Mariomeme.

Luigi always smiles, he knows he did the right thing, people from all over the world praise him, and call him saint and so on.
